Online Forms Instructions

Step 1: Download Adobe Acrobat Reader DC (If not already installed on computer)

Step 2: Download forms to your computer.

Step 3: Open forms in Adobe Reader.

Step 4: Click Fill and Sign and complete.

Step 5: Save as a different file name to your computer.

Step 6: Email your forms to your counselor.

Insurance Policy:

LFC does not accept insurance, however you can submit forms to your insurance company for possible reimbursement.
